Rhinitis Treatment
Rhinitis is one of the most common health problems in the world and can significantly impact quality of life, especially for those with comorbid conditions like asthma. In allergic rhinitis, the nose reacts to certain substances (called allergens) that trigger symptoms by releasing chemicals such as histamine. These substances are usually airborne particles that people inhale, such as pollen or mold spores. When these substances are inhaled, they enter the nose and lungs, where they cause inflammation and congestion. Antihistamines are a type of medication that stops the body from producing histamine and therefore can reduce or eliminate allergy symptoms. Several types of antihistamines are available, and they differ in terms of their frequency of use, delivery system (nasal spray or pills), cost, and side effects.
Another class of drugs, called leukotriene modifiers, can also reduce nasal inflammation and congestion by blocking the action of chemicals in the body that are similar to histamine. Examples include montelukast and zafirlukast, and they are often effective for patients who have both allergic rhinitis and asthma. Neither of these medications should be taken by women who are pregnant or breastfeeding.
For those with occupational rhinitis, the most important step in rhinitis treatment is to identify the source of the irritation and prevent exposure to it. A detailed history and physical examination is essential, and the patient should be interviewed to determine the nature of their job and possible exposures. Skin testing is generally not required for this purpose, but blood tests known as radioallergosorbent assays are used to assess levels of IgE antibodies to particular allergens.
Rhinitis may be caused by irritants, such as smoke or perfumes, which can trigger the immune system to produce IgE antibodies and cause an allergic reaction. Irritant-induced rhinitis may not respond to antihistamines and may require the use of other medications, such as intranasal corticosteroids. Some types of intranasal steroids are now available over-the-counter, such as fluticasone (Flonase Sensimist), loratadine (Claritin) and oxymetazoline (Naprosyn).
